Important information about a new Urgent Community Response service in Lincolnshire
Lincolnshire Community Health Services NHS Trust is launching a new Urgent Community Response service in October 2021.
Adults with complex health needs who have a very urgent care need, including the risk of being hospitalised, will be able to access urgent support from this new service within two hours. A multi-disciplinary team will provide patients with the care they need to remain independent and avoid hospital admission. The new service is community-based, supporting patients in their usual place of residence. The support available includes assessment and short-term interventions.
The Urgent Community Response will compliment a range of existing options already available via primary care home visits, East Midlands Ambulance Service NHS Trust or LIVES falls response and so on.
Health and social care professionals, voluntary and third sector partners, can make referrals by calling 0300 123 4868, option 8.
The service will operate Monday to Sunday, 8am to 8pm, with referrals accepted until 6.30pm.

We are rolling out our offer in phases and health, social care and third sector partners can contact Urgent Community Response service with their referrals from 1 November 2021.
