Social prescribing is about connecting people to a wide range of non-medical support within the community. Social Prescribing Link Workers have a conversation with people about what matters to them and help them take the next steps. In some areas, Social Prescribing Link Workers work alongside Health and Wellbeing Coaches to offer tailored support with specific personal goals.

To get in touch with social prescribing services you may need to contact your local Primary Care Network (PCN) or CVS Lincolnshire. CVS Lincolnshire provides social prescribing as part of the Mental Health Transformation Project, as well as on behalf of some Primary Care Networks (groups of GP surgeries) in Lincolnshire.

To make a referral for general social prescribing services you will need to know the name of the GP practice the person is registered with, or your own GP practice if you are making a referral for yourself.

  1. Use the list on the right to find the name of the GP practice and check which Primary Care Network (PCN) it is part of. Practices are listed in alphabetical order A-Z.
  2. Find the referral details for each Primary Care Network below.

People are encouraged to self-refer via their practice’s Anima system, stating ‘social prescribing self-referral’ as the reason for the request.
