Personalised Care, Support Planning and Shared Decision Making
Are you passionate about Personalised Care and have great facilitation skills? Would you like to become a ‘Train the Trainer’ in Personalised Care and Support Planning and Shared Decision Making as part of your existing role to support the training of staff across the Lincolnshire Integrated Care System? What would becoming a ‘Train the Trainer’ involve? You would be trained in Personalised Care and Support Planning and Shared Decision-Making modules by TPC Health and then trained on how to deliver them. You would then deliver the training with another trained practitioner via MS Teams to practitioners from across the Lincolnshire Health and Care system. |
How often would I be delivering the training? You would deliver each 3-hour session i.e., PCSP and SDM once a quarter with another trained facilitator |
Is the training accredited? Both modules are accredited by the Personalised Care Institute. |
When is the ‘train the trainer’ programme running? PCSP – 20th April – for delivery of three-hour session 28th April – for full day delivery (How to deliver) SDM – 16th May – for delivery of three-hour session 23rd May – for full day delivery (How to deliver) (You must be able to attend all the above sessions) |
Is the training on MS Teams or face to face? All of the training is via MS Teams |
I want to sign up. What do I need to do? Check you are available to attend all the training dates Discuss the opportunity with your line manager and ensure they are happy to release you for the training and then also to deliver the training to staff in the Lincolnshire ICS (this will be each 3-hour module once a quarter).
