The next Personalisation Huddle is 9.30-11am on Friday 3rd March and the focus is Supported Self-Management and ‘Patient Activation Measure’ (PAM).
People who are more confident and able to manage their health conditions (people with higher levels of activation) have 18% fewer GP contacts and 38% fewer emergency admissions than people with the least confidence. It therefore makes sense for practitioners to try and increase people’s ability to self-manage. PAM is a licensed tool that measures people’s knowledge, skills and confidence (referred to as ‘patient activation’) in managing their health and wellbeing. Join us to hear how different teams in LCHS and ULHT have been using Patient Activation Measure to tailor their conversations and/or interventions and the difference it’s making.
