Age UK Lincoln & South Lincolnshire launches I Forget dementia support service
It with a great deal of excitement and pleasure that I write to inform you that Age UK Lincoln & South Lincolnshire (AUKLSL), in partnership with Lincs Independent Living Partnership (LILP) a consortium consisting of AUKLSL, St Barnabas Hospice, Lace Housing Association and LHP, are delighted to announce the launch of our brand new service, I Forget. A service designed and co-produced with people that are living with dementia and their carers for, not only those diagnosed with dementia but also for anyone who is experiencing memory loss.
Having listened to what it is that people living with dementia and their carers have told us they would find useful, we have developed a service, funded from our own resources, to provide the following initially:
- Information Hub
- Education
- Case Management
- Social Activities and Inclusion
- Advance Care Planning
- Crisis Avoidance
- Support before and after diagnosis
The above is not an exhaustive list, the service will grow and develop as we learn from those accessing it, be that people with memory loss or dementia, their friends and family or system partners from across our ICS.
We will open to referrals on Monday 3rd July and the service will operate Monday-Friday between 08:45-4pm each day. Attached are documents that provide further information and details of how to make a referral.
We will be working with system partners from across the ICS but are very pleased to be working closely with LPFT colleagues Steve Roberts, Associate Director of Operations – Older People & Frailty Divisionand SRO for dementia and Gina Thompson, Dementia Transformation Programme Lead, to ensure a really smooth interface between our service and theirs, in order to provide a ‘no wrong door’ approach to this area of work and support the pressures within our service areas, aiming for people to receive the right level of support when they need it.
We will be officially launching the service later in the year where we plan to share our early findings and raise further awareness about the service, its impact and the learning we will no doubt have by then, so watch your inboxes for a save the date coming out soon.
