Social Prescribing
Social prescribing is about connecting people to a wide range of non-medical support within the community. Social Prescribing Link Workers have a conversation with people about what matters to them and help them take the next steps. In some areas, Social Prescribing Link Workers work alongside Health and Wellbeing Coaches to offer tailored support with specific personal goals.
As of August 2025 the way social prescribing referrals are made in Lincolnshire has changed.
To make a referral you will need to know the name of the GP practice the person is registered with, or your own GP practice if you are making a referral for yourself.
